Heart Failure ICP Programme

Our Heart Failure ICP Programme is helmed by a multi-disciplinary team comprising a cardiologist, case manager, pharmacist, physiotherapist, occupational therapist and dietitian.

The programme involves collaborations with primary care physicians to ensure smooth continuity of care when patients are discharged back into the community.

Heart failure patients will be able to visit our GP partners who are conveniently located within their neighbourhood. Potential heart failure patients may also be referred by the GPs for specialist management.

Only direct referrals from JurongHealth Campus' GP partners will be accepted for our Heart Failure ICP Programme.
