Multi-disciplinary Care Services

With the aim of designing ideal care for our patients and enhancing our service delivery, JurongHealth Campus offers multi-disciplinary care (MDC) services at Ng Teng Fong General Hospital.

These are run by multi-disciplinary teams comprising two or more specialties or professions. The members of each multi-disciplinary team work together to develop a common care goal, care plan and communication plan for patients. Care coordination and communication will be enabled across care providers to reduce duplication of care and cut down on the number of visits for patients.
