Community Outreach
To encourage the community to lead a healthy lifestyle, NTFGH, together with Ms Low Yen Ling, Mayor of South West District, visited the seniors at Lion Befrienders Senior Activity Centre (SAC) this morning for the graduation ceremony of a six-week workshop - CDSMP (Chronic Disease Self-Management Programme).
Equipping residents afflicted with chronic diseases with knowledge for better management of their conditions, CDSMP @ South West is conducted by Allied Health Professionals from NTFGH and JCH.
At the event, past CDSMP participants joined in to celebrate the successful outcomes of the previous runs and to help encourage active living among the residents.
Said Ms Low, "Chronic diseases is on the rise in Singapore and is a worrying trend. I am pleased that CDSMP has shown positive outcomes and reported a high level of self-efficacy in disease management and self-reported health status. Riding on the success, we intend to roll out a total of seven runs of the programme for FY2018/2019, and want our residents to know that they are not alone in this journey towards a healthier life. I wish to thank NUHS and the Allied Health team from JurongHealth Campus; and our supporting SACs and CCs in facilitating and managing CDSMP at South West for our residents. Together, we can fight the threat of chronic diseases."
